School Registration Info.


New Registration Guideline:

Attention All Families: Beginning with the 2017-2018 School year all students must submit the required Health Forms on or before the 1st day of school or they will NOT be allowed to start school until it is complete.

Students entering Kdg, 6th and 9th Grades are required to submit a School Physical with Up-to-date immunizations, or Physician signed Medical or Religious Objection paperwork. 12th graders must be Up-to-date with two Meningitis immunizations, or one after their 16th birthday.

This change is being implemented following School Board adoption of this New 1st Day Exclusion Policy at their February 21, 2017 meeting. This is a change from the October 15exclusion policy which was being followed previously.


***Free lunch update.
If your student receives free lunch due to receiving SNAP, TANF or Medicaid benefits their
registration fees for the school year are automatically waived! You do not need to fill out a fee waiver form. When you receive your letter stating that you student is eligible for free lunch due to one of the benefits listed, if there are other students in your household please contact your student’s school and or Janice Burmeister, (Food Service Director) to have them added for free lunch and waived fees. —

Registration Information – NEW Students

Kindergarten students MUST be five years old on or before September 1 in order to start school.

Proof of Residency

Each family must prove residency in the district each year. When coming to registration, bring items to prove residency. You will NOT be able to register prior to proving residency:
Proof of Residency: Provide TWO of the following:
  • Most recent property tax bill and proof of payment, e.g., canceled check or Form 1098 (homeowners)
  • Mortgage papers (homeowners)
  • Signed and dated lease and proof of last month’s payment, e.g., canceled check or receipts (renters)
  • Letter from manager and proof of last month’s payment, e.g., canceled check or receipt (trailer park residents)
  • Letter of residence from landlord in lieu of lease (Complete Form 7:60-E2)
  • Current public aid card
  • Current homeowners/renters insurance certified issues within 90 days
  • Current gas, electric, or water bill (Telephone/cell phone bills are NOT accepted) If two utility bills are used, they must be from two different utilities issued within the last 30 days.
Military Personnel Enrolling a Student for the First Time in the District must provide one of the following within 60 days after the date of student’s initial enrollment:
  • Postmarked mail addressed to military personnel
  • Lease agreement for occupancy
  • Proof of ownership of residence
Military Personnel Wanting to Keep Child/Ward Enrolled in the District Despite Having Changed Residence Due to a Military Service Obligation. Upon submitting a written request, the student’s residence will be deemed to be unchanged for the duration of the custodian’s military service obligation. The District, however, is not responsible for the student’s transportation to or from school.

What are the 5Essentials and 5Essentials Survey?

5Essentials is an evidence-based system designed to drive improvement in schools nationwide—it reliably measures changes in a school organization through the 5Essentials Survey and provides individualized, actionable Reports for each school. The 5Essentials system is based on more than 20 years of research by the University of Chicago Consortium on School Research on five components found to be critical for school success:

  • Effective Leaders: The principal works with teachers to implement a clear and strategic vision for school success.
  • Collaborative Teachers: Teachers collaborate to promote professional growth.
  • Involved Families: The entire school staff builds strong relationships with families and communities to support learning.
  • Supportive Environment: The school is safe and orderly. Teachers have high expectations for students and support students to realize their goals. Classmates also support one another.
  • Ambitious Instruction: Classes are academically demanding and engage students by emphasizing the application of knowledge.

The 5Essentials Survey is taken by all prekindergarten through 12th-grade teachers and all fourth- through 12th-grade students and usually requires no more than 30 minutes to complete. The information collected through the survey is rigorously reviewed and analyzed to generate a 5Essentials Report for each school. The 5Essentials Report includes a breakdown of teacher and student responses and, most importantly, provides a comprehensive picture of the school environment based on five essential areas critical for school improvement in a meaningful context of similar and successful schools.
