Health-Related Examination Requirements
 
Preschool – Physical exam with updated Immunizations within the last calendar year prior to the first day of school (Students new to preschool only)
 
Kindergarten – Physical exam with updated Immunizations within the last calendar year prior to the first day of school. Vision exam is highly recommended.
 
6th Grade– Physical exam with updated Immunizations within the last calendar year prior to the first day of school.

9th Grade-Physical exam with updated Immunizations within the last calendar year prior to the first day of school.

12th Grade– Meningitis two doses of meningococcal conjugate vaccine at entry to 12th grade.(Only one dose is required if the first dose was received at 16 yrs of age or older.)

New Students – Physical exam with updated Immunizations within the last calendar year prior to the first day of school.
