Pfizer vaccines for North Boone students aged 12 and older and their families now available for a clinic on May 19, 2021.

Time slots: Students aged 12 and older and their family members may be vaccinated at anytime, please note the locations by time:

  • North Boone Middle School 7:30AM – 9:30AM

  • Belvidere High School 10:30AM – 1:30 PM

If your child has special needs and requires their vaccine be administered in a private area, we are more than happy to provide this service. Please let one of our volunteers know when you check-in at the clinic.

  • There will be a limited supply from the Boone County Health Department.

  • Vaccines will be given by appointment only. Please sign up for an appointment by visiting the links above.

  • Students who are not 18 will either need a guardian present or a guardian-signed consent form.

To be vaccinated, students must bring:

  • School ID with photo to validate time slot

  • State ID or Driver’s License (if you have one)

PFIZER FACT SHEET FOR RECIPIENTS AND CAREGIVERS

The following must occur before vaccination will be administered:

  1. Consent forms MUST be signed and returned WITH THE STUDENT on the day of vaccination OR the parent/guardian MUST be on site

  2. Tickets with the QR code, either printed or on mobile device MUST BE WITH THE STUDENT

  3. NO VACCINE WILL BE GIVEN UNLESS THE STUDENT HAS A SIGNED CONSENT FORM (OR THE PARENT/GUARDIAN IS ON SITE) AND A TICKET WITH THE QR CODE
